The City of Bend Police Department would like to provide patches to all requesters, however, our policy only permits us to send a patch to active or retired law enforcement.

If you meet this criterion, please complete the information below and attach a copy of your current law enforcement employee photo identification card, or your retired law enforcement employee photo identification card.
