Photo Release Forms

All participants must also have a Photo Release Form for all on-camera actors/actresses.

Please share the link to the Photo Release Form with each actor/actress in your video so they may sign and submit the form independently of your application form. A parent’s signature is required for participants under the age of 18. Participants over the age of 18 may sign for themselves.
